1) Eligibility: You may obtain a free Music Maze entry form and official rules by sending a self-addressed stamped envelope with request clearly printed on a 3-1/2"x5" card to Sweet! Music Maze Contest Entry Request, c/o Sweet! Digizine, 148 S. Victory Blvd., Burbank, CA 91502. Mailed requests must be received by Feb. 1, 1997. Faxed requests will not be accepted. The Music Maze Contest is open to legal residents of the United States and Puerto Rico. Employees of eΓÇóworks, Inc., Sweet! Digizine, Motivational Media Assemblies, their affiliates, subsidiaries, and advertising, promotion, judging and other coordination agencies involved in this promotion and the immediate families of each are not eligible to participate. Void where prohibited.
2) To Enter: Correctly name all of the movie titles in the Music Maze Contest. Answer the questions on the entry form, include your name, address and phone number and mail your completed entry form to Sweet! Music Maze Contest, c/o Sweet! Digizine, 148 S. Victory Blvd., Burbank, CA 91502. Only one entry per person is allowed. All entry forms for the Music Maze Contest must be received by Feb. 1, 1997. The winning entry form will be selected by a random drawing from all eligible entries received, to be held by eΓÇóworks, Inc. on or about Feb. 5, 1997, with the decision of eΓÇóworks, Inc. being final. The winner will be notified by telephone or by mail. Prize claims must be received by June 1, 1997. All entry forms submitted become property of eΓÇóworks, Inc. and will not be returned. Entry forms subject to verification before awarding of the prize by eΓÇóworks, Inc., whose decision is final
3) General: The winner will be required to sign and return an affidavit of liability/publicity release within a specified time after notification or the prize will be forfeited. By accepting the prize, winners consent to the use of their name and/or likeness in advertising and promotion materials without additional compensation. Unclaimed prizes will be awarded or not awarded at the discretion of eΓÇóworks, Inc. eΓÇóworks, Inc. is not responsible for late, damaged, incomplete, illegible, postage due, lost, delayed, mutilated, or misdirected entry forms or submissions. One prize of 100 compact discs will be awarded. Only the prize listed in the preceding sentence will be awarded. Prizes are non-transferable. All taxes are the sole responsibility of the winner. No cash equivalents or substitution awarded for the prize, except at the discretion of eΓÇóworks, Inc. The odds of winning the prize depends on the number of eligible entries received.
